Is there any way to have the opt in field for the email address to be anywhere else other than the columns? Like say in the header or footer, or even on a custom html page? Thank you.

Custom HTML page, no, but the header or footer you could technically be accomplished using the nsScript call:

CODE
<ns:MailingList />


But you will need to perform some CSS wizardry to get it to look like it isn't a module in the header.
