Since Search Engine Optimization is never an exact science, I always take any scan results like these with a grain of salt and look at them carefully for their accuracy and the benefits they can give you. I can't answer all of your questions, but I did want to mention one detail.

The main product image in our software does indeed have ALT information and you should definitely make use of it. I note that the scan results you quoted above states that your keyword is not present in the first 3 ALT image attributes, but it does not specify that any of those 3 are your main product image. It may be looking at your logo or your navigation images or your category module headers.
I checked this product -
http://www.basstackledepot.com/spro-little-john-md.aspx - and I can see that the main product image does in fact have an ALT element, but it does not look like you have entered anything.
